We at Covenant Insurance Group understand that medical information regarding you and your health is personal, and we are committed to protecting this information. This information is used only for the purposes of affecting, administering, enforcing and servicing your benefit plan through Covenant. We maintain physical and procedural safeguards to ensure that your information is treated responsibly and in accordance with our privacy policy. We do not disclose any non-public information about you to any affiliated or non-affiliated third parties, except as required in performing the aforementioned duties, and your personal information is never used for marketing purposes.

What do we consider non-public/private information?

• Name, address, birth date, e-mail address, phone number, social security number, and drivers license number
• Personal Medical Records
• Underwriting information that we receive as a result of insurance applications
• Client information received from claims processing
• Any and all client insurance information

Who has access to this information?
In order to best serve our clients, it is Covenant Insurance Group's policy to allow their employees, and certain other companies working on our behalf, to have access to this information. In addition, we may provide information about you to other insurance organizations and companies to perform functions related to insurance business involving you. With proper identification and written request, clients may view the information regarding them contained in our files.

What steps do we take to safeguard your privacy?

• We limit the collection and use of customer information to the minimum we require in the process of delivering and administering our client's insurance.
• Only authorized agents and staff who need to know your personal information will access it and use it.
• We will not share personal client (either current or former) information outside of Covenant Insurance Group for any purpose other than the underwriting or administration of a client's policy.
